Overview

The Applicant Intake system captures structured data about applicants to enable intelligent resource matching.

Purpose

Creates a Universal Applicant Profile that contains information needed to:

  • Match applicants with appropriate resources
  • Prioritize applicants based on vulnerability
  • Track household composition and needs
  • Support coordinated entry workflows

Multi-step Form

The intake form has 7 steps:

  1. Head of Household - Name, contact, demographics
  2. Household Demographics - Size, composition, location
  3. Income & Employment - Income sources, employment status
  4. Housing Situation - Current housing, history
  5. Immediate Needs - 14 need categories
  6. Document Upload (Optional)
  7. Review & Consent

Vulnerability Scoring

Automatic scoring (0-100) based on:

Priority Levels

  • Urgent (70-100): Immediate intervention required
  • High (50-69): High priority for services
  • Medium (30-49): Standard priority
  • Low (0-29): Lowest priority
